Prince Chapman Academy will be hosting a flu vaccine clinic for its students on October 9 during school hours. If you would like your student to receive a flu vaccine on this day, please complete this form at this link: https://supershot.org/consent-for-services/ Electronic registrations should be submitted by September 27. We will only offer the nasal mist for students, as it is more easily administered and emotionally tolerated by youth. Families can also register on paper by filling out a form which is available in the office. If you would like for your student to bring one home, please contact the school. These need to be brought back to the office as soon as possible after completing.
